Documents Required for Visa Medical Examination Services
- Application Form
- Passport (Original)
- Hong Kong ID card (Original)
- Photocopy of passport or identity document
Applicants below age of 18 will need to be accompanied by a parent or legal guardian who must bring the following
- HKID card / passport (Original)
- Birth certificate / Legal guardian certificate (Original)
- Photocopy of parent or legal guardian’s passport or HKID, birth certificate or legal guardian letter
Please provide your COVID-19 vaccination certificate if you have been vaccinated for one or more doses. At present it is not mandatory to have the COVID-19 vaccine before your medical examination.

Basic Check-up Items
On the day of the examination, depending on your Health certificate type you will have
- Chest X-ray if aged 11 or above
- Physical examination
- Urine test for applicants aged 5 or over
- A blood test may also be needed depending on your visa type and health condition
