What is DHC?
The Chief Executive advocated the set up of District Health Centres with a brand new operation model to provide primary healthcare services through medical‑social collaboration and public‑private partnership. The aim is to:
- enhance public awareness of disease prevention and their capability in self-management of health;
- drive healthy lifestyle for prevention of chronic disease;
- support the chronically ill to prevent deterioration; and
- enhance client access to primary healthcare services.
Interim DHC Express will also be established to provide health promotion, consultation and chronic disease care services.
Who is eligible to receive this service?
- An individual above 11 years old with Hong Kong Identity Card issued under the Registration of Persons Ordinance (Cap. 177, the Laws of Hong Kong) or the certificate of exemption; and (Except those who obtained their Hong Kong Identity Cards by virtue of a previous permission to land or remain in Hong Kong granted to him and such permission has expired or ceased to be valid),or a child who is a Hong Kong resident and under 11 years of age;
- agrees to enrol in the electronic Health Record Sharing System (eHRSS); and
- is living or working in the district of the corresponding DHC.
How to register as member?
Eligible individual can register at corresponding DHC or seek referal from corresponding service units.
Are there any charges for DHC services?
Registration and membership are free.
All of the group activities are free, as well as the individual healthcare services provided by nurse, pharmacist and social worker at the DHC.
Individual healthcare services including medical consultation, physiotherapy, occupational therapy, dietetics, optometry assessment, speech therapy, podiatry, Chinese medicine services and medical laboratory services require co-payment by the DHC client.
How to use the services of Quality Healthcare?
As one of the largest private healthcare providers in Hong Kong, Quality HealthCare provides comprehensive referral services for the first DHC in Hong Kong. We offer Western Medicine, Physiotherapy, Traditional Chinese Medicine (Acupuncture and Manual Therapy) and Diagnostics & Imaging services to help the participants to manage and identify chronic disease including hypertension, diabetes, low back pain and osteoarthritic knee pain.
Participants will first conduct a Health Risk Factors Assessments at DHC. Individuals will be referred to Netword Doctor for medical consultation and diagnosis services as needed. The service fees will be subsidized by the Government , and participants would only require to pay the co-payment according to the type of service.
